Entering Student Information Form (download here)

There is no application to join the Bailey Scholars Program. Students self-select themselves into the program and, if they find it doesn't fit into their collegiate journey, can self-select themselves out. Thus, an informational form is necessary for each new student to begin the administrative process of incorporating the student into the program, not as a method of selection for the program.

Learning Vision Statement Sheet (download here)

This sheet is to help undergraduates begin thinking about their Learning Vision Statement. They may add or detract from their Learning Vision Statement at any point in their Bailey Scholars Program experience as they see fit.

Middle Twelve Experience Sheet (download here)

This form must be filled out (the general information and the first question) and turned in at the beginning of the semester an undergraduate is taking her/his Middle Twelve course. It must be approved by the Senior Director, and must relate to the student's Learning Vision Statement. At the end of the course, the student will fill out the remaining questions. By the following semester's end, the student must also share this experience back with the community in a way that best fits the student.

Co-Curricular Experience Sheet (download here)

This form must be filled out and turned into the Senior Director in order for a student to count a Co-Curricular experience as part of her/his learning portfolio at the end of the experience. It must relate back to the student's Learning Vision Statement in order for it to be considered part of her/his learning portfolio. By the following semester's end, the student must also share this experience back with the community in a way that best fits the student.

Student Learning Allowance Request Form (download here)

The Bailey Scholars Program has had the privilege for a number of years of having a budget which allows for learning allowances for student scholars to help support them in their learning journeys. This money can be used in a wide variety of ways, such as to attend a conference, purchase books or resource materials, or take a workshop or course outside MSU. It is expected that each scholar will respect this resource by choosing appropriate activities or resources and also by sharing their learning through this investment back with the community at large. This will be discussed with the Senior Director during the request process. The Senior Director will determine with the BCC the availability of funds each year. To request funding up to $100, please follow these steps:

  • Generate an idea
  • Discuss how this idea fits with your envsionment with the Senior Director
  • Complete and submit this form and the Funding Request form to the Senior Director together
  • Work with the Bailey Secretary and Senior Director to arrange for funding disbursement

    To request funding from the Bailey Scholars Program either as an individual, internally, as a course, or as an external organization/individual this form must be submitted and approved by the Senior Director. This form must accompany any learning allowing request forms.

Bailey Scholars Specialization Progress and Planning Form (download here)

This form is for students to track their progress in their Connected Learning Specialization, and to plan for future experiences.

Bailey Scholars Student Scholars Questionnaire (download here)

This information is gathered on each student in the Bailey Scholars Program to better understand the needs and population of our undergraduate students. It is also used for information and assessment reporting as well.

Academic Advising Notes Form (download here)

This form is used by both the Senior Director and students in academic advising appointments to take notes and move forward on action items.
